Objective: Narrative objectives1 - The aim of this study is to conduct a prospective survey of patients attitudes toward participation in outpatient cardiac rehabilitation, to identify barriers to participation, and to utilize this information for individualized support. Furthermore, we aim to examine the relationships between actual participation/continuation status and each barrier and to feed the findings back into real-world clinical practice. Intervention: Interventions/Control_1 - Questionnaire survey regarding participation in outpatient cardiac rehabilitation

Eligibility: Age-lower limit - 20 years-old 20 years Require inpatient treatment in the Department of Cardiology at Kasugai Municipal Hospital for acute heart failure or acute exacerbation of chronic heart failure Not enrolled in Long-term Care Insurance at the time of admission Key exclusion criteria - Patients who meet any of the following criteria: Severe hearing impairment or dementia that makes communication difficult Refusal to participate in the questionnaire-based intervention Unsuitable by the principal investigator or a co-investigator Target Size - 200
