Narrative objectives1 - To develop a Nursing Practice Model to Promote Advance Care Planning for Stroke Survivors at home,and to Evaluate the Effectiveness and Feasibility of a Nurse-led General Advance Care Planning Booklet for Stroke Survivors at home

Intervention:
Interventions/Control_1 - Home care nurses will provide support to stroke survivors using a ACP booklet based on the nursing practice model on three occasions during regular home care visits.A questionnaire survey will be administered to the stroke survivors before and after the intervention. After the intervention, interviews will be conducted with the subjects (stroke survivors and nurses who provided support) whose consent was obtained.

Key inclusion criteria - Nurses: 5-10 individuals meeting the following criteria
1 Working at a home-visit nursing station
2 At least three years of clinical experience as a nurse
3 Able to communicate in Japanese
Stroke Survivors: 10-20 individuals meeting the following criteria
1 Those who have been diagnosed with a stroke (with a history of cerebral infarction, cerebral hemorrhage, or subarachnoid hemorrhage; the number of years since onset is not specified)
2 Utilizing home-visit nursing services
3 No apparent decline or loss of cognitive function
4 Able to communicate in Japanese
Key exclusion criteria - 1 Apparent decline or loss of cognitive function
2 Inability to communicate in Japanese
